PDF with only the first level of bookmarks open

Using current master, mupdf-gl displays all bookmark levels. With the attached
PDF document, it should only display the two first levels. This means, to open
the first level and to close the second.

If you want characters for closed an open bookmarks with descendants, NotoSerif
has ►▼.

In the attached image, mupdf-gl displays all bookmark levels (I.III is not even
displayed).

xpdf displays the bookmarks levels as specified in the document.

The PDF Specification explains the way of determining open and closed bookmark
levels in page 376 (page label 368).

It is the key /Count in the outlines dictionary and each of it entries.
